RALLEE^® Certified Genetic Testing Requisition Form

All orders must have a completed requisition form to be processed by MoxxiTech^®. Completed forms may be returned by email to orders@moxxitech.com or may be included with samples shipped to our laboratory.

Please note: we request that all samples be drawn by a veterinarian for your safety and the safety of your animals. 

However, we require samples provided for Custom Genetic Testing to be drawn by a veterinarian and samples must also be accompanied by a completed RALLEE Requisition Form, including all relevant medical charts.
